Spain is a country with a high level of social welfare. Every Spanish immigrant is entitled to social health insurance. Spain's healthcare system not only ensures that all citizens enjoy a fair medical system but also improves the efficiency of the public healthcare system by actively introducing market factors into the medical service system to promote efficiency through competition.

1. What types of health insurance are available in Spain?
Insurance in Spain can be divided into social insurance and commercial insurance.
Social insurance or public insurance means that as long as you are employed and insured, you are entitled to unemployment benefits, subsequent pension insurance, and social health insurance. Applications for unemployment benefits and pensions are calculated based on the insurance contribution base and the number of days insured, while social health insurance is a social insurance service that you can enjoy as long as you are employed and insured, or if your economic dependent is employed and insured.
Commercial insurance or private insurance means that you can choose an insurance company and purchase a type of commercial insurance based on your personal insurance needs, such as life insurance, car insurance, or medical insurance.
2. What are the main components of health insurance?
Social health insurance is comprehensive insurance that covers any medical condition. Commercial insurance depends on the services provided by each individual company.
3. How do I apply for a medical card? Does it fall under social insurance or medical insurance?
The public medical card is a medical card that can be enjoyed under social insurance. The application process is very simple: as long as the applicant holds a residence permit and is employed, or their dependent is employed, or they are unemployed but receiving unemployment benefits, they can go to the social security office to print a social security certificate (el reconocimiento del derecho a la asistencia sanitaria). Then, take the certificate of residence (empadronamiento) and a copy of the residence permit to the clinic nearest to your residence to apply. You will immediately be issued a temporary medical card valid for three months. The actual medical card will be mailed to your residence within those three months.
4. What is the validity period of the medical card, and how do I renew it?
The validity period of the medical card generally follows your residence permit. Therefore, we recommend that you apply for the renewal of your medical card within two months before your residence permit expires. The required documents are the same as the initial application list: the original and a copy of your residence permit, the certificate of residence, and the social security certificate printed by the Social Security Bureau. You can process this at the health center (centro de salud) in your residential district.
5. What is the nature of the current healthcare system in Spain, and is it valid nationwide?
The validity of social insurance while working in Spain is nationwide. However, it should be noted that you must register at the clinic nearest to your registered address, and the primary diagnosis location is generally that clinic.
On September 20, 2013, a "nationwide unified medical card" was officially approved. This means that this new type of medical card will be usable in all public medical clinics and hospitals throughout Spain. Furthermore, the government plans to fully transition the current regional-based medical cards to nationwide universal medical cards within the next five years.
